The "SUBMIT" button on the Unified-Self Service (USS) request form is getting grayed out and is not sending the request

Issue:
When you create a request on the USS the "SUBMIT" button does not work, it simply turns grayed out.
Resolution:
1 - Open the USMHOME/view/webapps/usm/WEB-INF/web.xml file.
2 - Modify the Current Key with the New Key as shown next.
Current Key:
<filter>
<filter-name>CorsFilter</filter-name>
<filter-class>org.apache.catalina.filters.CorsFilter</filter-class>
<init-param>
<param-name>cors.allowed.origins</param-name>
<param-value><http/https>://<Catalog Host Name>:<Catalog Port No>,<http/https>://<USS Host Name>:<USS Port No></param-value>
</init-param>

New Key:
<param-name>cors.allowed.origins</param-name>
<param-value>*</param-value>

3 - Save the file and clear the Catalog environment by running:
4 - Stop the Catalog service
5 - Empty/clear the %USM_HOME%\view\translets folder
6 - (re)move the %USM_HOME%\logs\*.log files
7 - Clear the browser-cache
8 - Start the Catalog service
9 - Restart the USS service(s) (CA Unified Self-Service Server (and Jetty Server))
