TAI not initializing

Issue:
Using the 12 SP2 WebSphere Application Server Agent, it is unable to initialize when using a Cluster type HCO of policy servers.  The error message displayed is:

Trust Association Init Error. The Trust Association interceptor implementation com.netegrity.siteminder.websphere.auth.SmTrustAssociationInterceptor initialization failed. The error status/exception is java.lang.IndexOutOfBoundsException: Index: 1, Size: 0 


Another startup issue is a rather silent startup failure, which can be caused by an incorrect ACO being used that is missing settings that the WebSphere agent requires.
Environment:
12 SP2
All supported WebSphere versions and platforms.
Resolution:
A "Need It Now" build is available to resolve this issue, please contact support and request the NIN build package for this agent.

If the TAI agent is rather silently failing to initialize, review the ACO used and make sure it is based off of the ASAWASDefaultSettings ACO template.
