Receiving Sporadic Error on Workflow

We have a process that send an action item for idea to get approved and sometimes process is failing on an manual action that attempts to create an action item. When checking the process error message we see below error.
Receiving the following error when executing some of the workflows: com.niku.union.odf.exception.ODFValidationException: System error. Upon reviewing bg-ca.log we saw an error below indicating that data it was trying to insert is too large for that filed. Description field for an Action Item message is 2000 character limit, so you would need to ensure what is put into description does not go over that limit.
