Please provide implementation document outlining a step by step sequence of upgrade from 11.6 to 12.

You have now completed the preparation steps and are ready to install. You have two choices on how to install-  Either using CA CSM or downloading a PAX file from support.ca.com and installing from there.
I am not going to include all the details from the manual for each, but here is an overview:
  1. Installing  XCOM r12 using CA CSM:

  1. Acquire a new product.
  2. Install the product.
  3. Maintain the installed products.
  4. Deploy the product to the destination system.
  5. Configure the deployed product.

    2. Acquire the Product Pax Files

    3. Create a Product Directory from the Pax File
    4. Copy Installation Files to z/OS Data Sets
    5. Prepare the SMP/E Environment for a Pax Installation
    6. Run the Installation Jobs for a Pax Installation
    7. Clean Up the USS Directory
  2. Apply Preventative Maintenance  (Very important)
    1. Receive Maintenance and Error HOLDDATA
    2. Apply and Accept Maintenance
    3. Apply CA Recommended Service (CA RS) Maintenance

Now it is time to bring up XCOM.
  1. Starting your product:
    1. Configure CA XCOM Data Transport for zOS
    2. Set and Define the Language Environment Runtime Options (Optional)
    3. Generate Exits and Tables used by CA XCOM Data Transport
    4. Reassemble the CA ACF2 Security Module (CA ACF2 Security Users Only)
    5. Define the Libraries and Install the TSO/ISPF Facility
    6. Execute CAIRIM to Install LMP (Non-MSM Install Only)
    7. Define/Migrate the Request Queue
    8. Define/Migrate the VSAM History File
    9. Define the Optional Sequential Files
