IHS Web Agent not Working

Issue:
Although the Web Agent is initializing and logging, the browser is unable to connect to the IHS web server.
Cause:
IHS (Apache) was logging two errors to the Event Viewer's Application Log each time the server started.  Both errors were the same, stating the sm_module had already been loaded.  This caused us to examine the httpd.conf file and we found both the sm_module and SmInitfile were specified three times.
Resolution:
Deleting the two duplicate sm_module and SmInitfile lines and restarting the web server resolved the problem.
