Issue:
After implementation SLCM 14.1.0.5 (CP05), blank screen is displayed when a request is submitted.
Environment:
CA Service Catalog 14.x
Cause:
CP05 Patch implementation
Resolution:
Please contact support.

Once you have encountered the issue the current resolution is to install:

1.T6D9367 .CAZ on top of 14.1.05

Note: This fix was released as "Small Combo fix after 14.1 CP5"
