Solution:

This issue occurs because the Version Control in R12.1 is set to UPGRADE by default. To resolve the issue, disable the version control in Service Desk by performing the following steps:

  1. Login to SD as an administrator.

  2. Go to Administration tab -> Options manager -> Ver Ctl.

  3. Click and edit Ver Ctl and change the Option Value to DISABLE [DISABLE].

  4. Recycle services of SD.

  5. Now on NSM server, click start -> programs -> CA -> Service Desk. Click on Configuration.

  6. Once completed, manually start event convertor manually from servies.msc.
