CA1 & HSM ,stuck with tape request - I/O / close exit

Recently we move from Zos 2.1 to 2.2 and CA1 v14 was the same , got some stuck in all Zos , investigating more seems HSM and CA1 was in some Issues. An IPL should be done due system was very slow , HSM did not response and many jobs were in pending , running but without I/O . 
Because the device was NOT-READY at the time that IBM called the TEP-Close exit; our attempt to issue an EXCP (to read the buffered log for the tape device going through CLOSE processing) never completed - and our WAIT for the I/O to complete never came back. Because this requires an I/O error that prevents CLOSE from completing, I doubt it is something that can be duplicated; however I believe that RO96826 will prevent this going forward. With RO96826 we made some major changes and went back to our old hooks (the hooks used in previous releases) instead of relying on the TEP-Close exit. There are some additional fixes that should be applied; in addition to RO96826 you will need RO98235, SO03703 and SO03575. With these 4 solutions, I believe that if such an I/O error happens again it will not cause HSM to hang.
