CA Directory policy store failed to failover

Customer is using CA Directory as policy store with two replicated CA Directory servers in a failover configuration.

Users were unable to log in to an application.

It was discovered that CA Directory had stopped on one server, but it had not failed over to the failover server.
Customer had used commas instead of spaces in the LDAP IP Address field (on Data tab of smconsole).
