Attachment sync issue using Report and issue service

Issue:
when we create a request using service "report an issue" and we attach both note and attachments, only the notes get synced and attachment failed to sync
errors with 
com.ca.usm.ruleEngine.action.sdm.CreateIncidentException: Adding the notes to incident failed. Reason:NO AHD03055:Data partition Pre-Update restriction on Request
    at com.ca.usm.ruleEngine.action.sdm.CreateIncidentAction.addActivityLogs
Environment:
CA Service Catalog 17.1
Cause:
Attachment Sync and Policy based approval error
Resolution:
Please contact support.
Once you have encountered the issue the current resolution is to install:

1.T6D9362.CAZ on top of 17.1.
