[ADA] Backing up application definitions

Introduction:
The user interface does not provide a way for customers to backup/export application definitions.

In order to back up these definitions, direct mysql queries need to be run.
Question:
How can the Application Delivery Analysis applications be backed up?
Environment:
Windows
Answer:
The customer can run the following query to export the following information into a .csv file:

Application Name
Port number
Assigned servers' IP address
Assigned servers' hostnames

Mysql Query:
